I have been thinking about your post all day. I know some have had a good experience with the family meal. The very detailed account of your experience mirrors ours. My D was 11 at the time. 
I asked this question to this group to see the holes, and commonalities in the system and where there can be improvements.
I will be bringing up these experiences and my own to illustrate what the families felt during that time to the community ED team as a whole soon. I think that the family's experience gets missed when they just have "checklists" to fill out. 
I thank you for your candour. I also feel so sad that this happened to your family and especially to your D.  I hope your D is doing well and that you are getting the care you all deserve. 
My hope and goal and task now will be to help our ED clinic, both IP and OP, to just "do better".


Food+more food+time+love+good professional help+ATDT+no exercise+ state not just weight+/- the "right" medicine= healing---> recovery(--->life without ED)
